> Foundayo (orforglipron) is a once-daily tablet taken with or without food — no meal-timing restrictions. The dose escalates in steps of at least 30 days each: 0.8 mg → 2.5 mg → 5.5 mg, with optional further steps to 9 mg, 14.5 mg, and the 17.2 mg maximum. In the ATTAIN-1 trial, patients at the highest dose lost about 11% of body weight on average over 72 weeks.

Foundayo (orforglipron) is a once-daily pill used along with a reduced-calorie diet and increased physical activity to help adults lose weight and keep it off. It is prescribed for adults with obesity, or adults with overweight who also have at least one weight-related health condition (such as high blood pressure, high cholesterol, or type 2 diabetes).

Foundayo belongs to a class of medicines called GLP-1 receptor agonists. It works by reducing appetite and helping you feel full sooner.

In clinical trials, patients taking Foundayo at the highest dose lost an average of about 11% of their body weight over 72 weeks, and more than half of those patients lost at least 10% of their body weight.[[2]](https://www.nejm.org/doi/full/10.1056/NEJMoa2511774)

## How to Take Foundayo

- Take one tablet by mouth once a day, at the same time each day.
- You may take it with or without food — there are no meal-timing restrictions.
- Swallow the tablet whole. Do not break, crush, or chew it.[[1]](https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=8ac446c5-feba-474f-a103-23facb9b5c62)

## Dose Schedule

Your dose will be increased gradually over time to help reduce side effects. Do not change your dose unless your healthcare provider tells you to.[[1]](https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=8ac446c5-feba-474f-a103-23facb9b5c62)

| Step | Dose | Duration |
|---|---|---|
| 1 | 0.8 mg once daily | At least 30 days |
| 2 | 2.5 mg once daily | At least 30 days |
| 3 | 5.5 mg once daily | At least 30 days |
| 4 (optional) | 9 mg once daily | At least 30 days |
| 5 (optional) | 14.5 mg once daily | At least 30 days |
| 6 (optional) | 17.2 mg once daily | Maximum dose |

Your provider will decide whether to increase your dose beyond 5.5 mg based on how well the medication is working and how well you tolerate it. The maximum dose is 17.2 mg once daily.

## How Well Does Foundayo Work?

In the ATTAIN-1 trial — a large 72-week study of adults with obesity — weight loss increased with dose, with the highest dose producing an average reduction of about 11% of body weight; more than half of patients at that dose lost at least 10%.[[2]](https://www.nejm.org/doi/full/10.1056/NEJMoa2511774) Earlier phase 2 data showed weight reductions of up to 14.7% at 36 weeks at the highest tested doses.[[3]](https://www.nejm.org/doi/full/10.1056/NEJMoa2302392)

## What to Do If You Miss a Dose

- If you miss a dose, take it as soon as you remember.
- Do not take two doses at the same time to make up for a missed dose.
- If you miss 7 or more days in a row, contact your healthcare provider. You may need to restart at a lower dose and work your way back up to reduce the chance of stomach-related side effects.

## Common Side Effects

Most side effects are related to the stomach and intestines and tend to happen during the dose increase period. They are usually mild to moderate and often improve over time. Common side effects include:[[2]](https://www.nejm.org/doi/full/10.1056/NEJMoa2511774)

- Nausea
- Constipation
- Diarrhea
- Vomiting
- Heartburn or indigestion

Tips to help manage stomach-related side effects:

- Eat smaller, more frequent meals instead of large ones.
- Avoid greasy, fatty, or spicy foods.
- Stay well hydrated — drink plenty of water throughout the day.
- Eat slowly and stop eating when you feel full.

## When to Call Your Healthcare Provider

Contact your provider right away if you experience any of the following:

- Severe stomach pain that does not go away (with or without nausea/vomiting) — this could be a sign of pancreatitis
- Persistent or severe nausea, vomiting, or diarrhea that prevents you from keeping fluids down — this can lead to dehydration and kidney problems
- Signs of an allergic reaction: rash, swelling of the face/lips/tongue/throat, difficulty breathing
- Signs of low blood sugar (shakiness, sweating, fast heartbeat, dizziness, confusion) — especially if you also take insulin or a sulfonylurea
- A lump in your neck, trouble swallowing, shortness of breath, or persistent hoarseness
- Signs of gallbladder problems: pain in the upper right part of your stomach, fever, yellowing of the skin or eyes

## Important Safety Information

- Do not take Foundayo if you or a family member has had medullary thyroid cancer (MTC) or if you have a condition called Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).[[1]](https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=8ac446c5-feba-474f-a103-23facb9b5c62)
- Do not take Foundayo if you have had a serious allergic reaction to it in the past.
- Foundayo is not recommended if you have severe gastroparesis (a condition where the stomach empties very slowly).
- Tell your provider about all medications you take. Some medications may interact with Foundayo, including certain antifungals, antibiotics, seizure medications, and cholesterol-lowering drugs.
- If you take birth control pills, Foundayo may make them less effective. Talk to your provider about using a backup method of birth control (such as condoms) for 30 days after starting Foundayo and for 30 days after each dose increase.
- If you are pregnant, planning to become pregnant, or breastfeeding, talk to your provider before taking Foundayo. Foundayo may harm an unborn baby and should be stopped if pregnancy is confirmed.
- If you have a planned surgery or procedure requiring anesthesia, tell your surgical team that you are taking Foundayo, as it may slow stomach emptying.

## Storage

Store Foundayo at room temperature. Keep it in its original container and out of reach of children.

## Frequently asked questions

### Do I have to take Foundayo on an empty stomach?

No — that's what makes Foundayo different from other GLP-1 pills. As a small-molecule medication, it can be taken with or without food, at the same time each day, with no fasting window and no restrictions on water.

### What is the maximum dose of Foundayo?

The maximum dose is 17.2 mg once daily. Not everyone needs it — your provider decides whether to increase beyond 5.5 mg based on how well the medication is working and how well you tolerate it.

### What if I miss several days of Foundayo?

If you miss one dose, take it as soon as you remember — but never take two doses at once. If you miss 7 or more days in a row, contact your provider before restarting; you may need to restart at a lower dose and work back up to reduce stomach-related side effects.

### Does Foundayo affect birth control pills?

It may make them less effective. Talk to your provider about using a backup method (such as condoms) for 30 days after starting Foundayo and for 30 days after each dose increase.
