At Imbue Health, this includes asynchronous consultations (questionnaires), secure messaging, and review of medical history.

I understand that in case of a medical emergency, I should dial 911 immediately and not rely on this telehealth platform.
I agree to provide accurate, current, and complete information about my health history and current medications to ensure safe treatment.
I understand that prescriptions are provided at the sole discretion of the provider and are not guaranteed.
