Missing one dose will not undo your progress. What to do next depends on which medication you take — here are the label rules for all of them.

Never take two doses at once, and never take an extra dose to "make up" for a missed one.[1][3] Doubling up sharply increases nausea, vomiting, and diarrhea without speeding up weight loss.

Why the Rules Differ by Medicine

The timing rules follow how long each drug stays in your body.

The weekly shots last a long time. Semaglutide and tirzepatide remain in your bloodstream for about a week — that's why they're weekly drugs. Miss a dose by a day or two and the level in your blood is still meaningful; a late dose simply tops you back up. The cutoff exists for the opposite problem: a late dose taken too close to the next scheduled one briefly stacks two doses, and the resulting spike in drug level is what causes severe nausea and vomiting. Wegovy requires at least 2 days between doses; Zepbound requires at least 3 days (72 hours).[1][3]

The daily pills work differently. A single missed day is a much smaller gap. Wegovy tablets: the label says simply skip and take the next dose tomorrow. Foundayo is more forgiving — take a missed dose late in the day if you remember, just never two in one day.[2]

Food rules matter for Wegovy tablets. Oral semaglutide must be taken on an empty stomach for proper absorption — if you remember your tablet at lunchtime after eating, taking it then would be largely wasted, which is another reason the label says just skip.[1][4] Foundayo has no food or water rules — take it any time, swallowed whole (never broken, crushed, or chewed).[2]

If You've Missed Several Doses in a Row

This is the situation where you should not simply restart at your old dose. Your body loses its tolerance during a break, and returning straight to your previous dose often causes severe nausea and vomiting.

Don't decide this on your own — call your prescriber's office before restarting after any gap longer than one dose. Obesity-medicine guidance supports individualized dosing decisions based on tolerability, and specialists generally consider restarting the escalation schedule the safer choice after an extended gap.[5]

Expect that early side effects — nausea, fullness, constipation — may return when you restart, then improve as your body readjusts. Smaller, more frequent meals, steady hydration, and avoiding fried or fatty foods help.

Changing Your Injection Day (Weekly Shots)

You can move your weekly shot for travel or convenience. The only rule is minimum spacing between the old dose and the new one:

• Wegovy pen: at least 2 days (48 hours) between doses
• Zepbound: at least 3 days (72 hours) between doses

Example: You take Zepbound on Sundays and want Wednesdays. Take Sunday's dose as usual, then take the next one that Wednesday — 3 days, which meets the minimum. Wednesday is now your day.[3]

The daily pills don't have this issue — keep taking Wegovy tablets each morning on an empty stomach, and Foundayo once daily any time.

Delaying a Dose Increase Is Allowed

These medicines are increased slowly on purpose — gradual step-ups are the main way GI side effects stay manageable. Staying longer at your current dose is often the right choice: the Wegovy label says to consider delaying escalation if a dose isn't tolerated, and the Zepbound label says to consider a lower maintenance dose.[1][3] A delay is not a failure — many people do well long-term below the maximum dose. Storage After a Missed Dose

If a missed dose means your medicine sits out longer than planned, the room-temperature clocks keep running — a Wegovy single-dose pen gets 28 days out of the fridge and a Zepbound single-dose pen or vial gets 21 cumulative days. When to Call Your Prescriber

Call before your next dose if any of these apply:

• You've missed 2+ weekly injections in a row, or about a week of daily pills (for Foundayo, 7+ consecutive missed doses means restarting lower)
• You're unsure what dose to restart at after a break
• Severe or persistent nausea, vomiting, or diarrhea after restarting — or you can't keep fluids down
• Severe stomach pain that doesn't go away, especially spreading to your back — possible pancreas or gallbladder trouble; urgent
• You take insulin or a sulfonylurea — restarting after a gap can change your blood sugar, and those doses may need adjusting
• You take birth control pills and are on Foundayo — it can reduce oral contraceptive effectiveness; use a non-oral or barrier method for 30 days after starting and after each dose increase[2]
• You're taking, or being offered, a second GLP-1 at the same time — combining them is not recommended

The Short Version

Weekly shots have a grace period — 2 days for Wegovy, 4 days for Zepbound — after which you skip and resume. Daily pills have essentially none: skip Wegovy tablets and take tomorrow's dose; take Foundayo late but never doubled. Never double up. And after any gap of more than one dose, call before restarting so your dose can be stepped back down safely.
