Oral Wegovy Dosing: How the 25mg Semaglutide Tablet Works

Oral semaglutide 25 mg is a once-daily prescription tablet (brand name: Wegovy) used for weight management in adults with obesity (BMI 30 or higher) or overweight (BMI 27 or higher) with at least one weight-related health condition, such as high blood pressure, high cholesterol, or type 2 diabetes. It works by mimicking a natural hormone called GLP-1 that helps control appetite and food intake. It is used along with a reduced-calorie diet and increased physical activity.
How to Take Your Tablet — This Is Very Important
The way you take this medication directly affects how well it works. Only about 1–2% of the medication is absorbed into your body, so following these steps carefully is essential:
- Take the tablet first thing in the morning on an empty stomach, after fasting overnight (no food or drinks other than water).
- Swallow the tablet whole — do not crush, chew, or split it.
- Use only a small sip of plain water (no more than 4 ounces / 120 mL, or about half a glass). Do not take with any other liquids besides water.
- Wait at least 30 minutes before eating, drinking anything else, or taking any other medications.
- Take only one tablet per day. Do not take extra tablets to make up for a missed dose. If you miss a dose, skip it and take the next dose the following day.
Eating, drinking, or taking other medications too soon can significantly reduce how much of the drug your body absorbs, making it less effective.
Dose Schedule — Starting Low and Going Slow
Your dose will be gradually increased over 12 weeks to reduce side effects:

If you have trouble tolerating a higher dose, talk to your provider. The dose can be temporarily lowered and then increased again when you feel ready.
How Is This Different From Rybelsus?
You may have heard of Rybelsus, which is also an oral semaglutide tablet. Here are the key differences:
- Purpose: Rybelsus is approved for type 2 diabetes management. Wegovy oral 25 mg is approved specifically for weight management.
- Dose: Rybelsus comes in lower doses (3 mg, 7 mg, and 14 mg). The Wegovy tablet goes up to 25 mg, which provides greater weight loss.
- Results: In the OASIS 4 clinical trial, the 25 mg Wegovy tablet led to an average weight loss of about 13.6% of body weight over about 15 months — similar to the Wegovy injection.[1]
- Same fasting rules: both Rybelsus and the Wegovy tablet must be taken on an empty stomach with a small sip of water, with a 30-minute wait before eating or taking other medications.
- Not interchangeable: these products are not substitutable on a mg-to-mg basis. Do not switch between them without your provider's guidance.
What Results Can I Expect?
In the OASIS 4 clinical trial (published in the New England Journal of Medicine), people taking oral semaglutide 25 mg along with diet and exercise changes experienced the following results at about 15 months:[1]

In addition to weight loss, participants also saw improvements in blood sugar, triglycerides, and C-reactive protein (a marker of inflammation). Among those who had prediabetes at the start of the study, 71% returned to normal blood sugar levels.
Weight loss typically begins within the first few weeks and continues over several months. Results vary from person to person.
Common Side Effects
The most common side effects are stomach and digestive issues, especially during the first few weeks and when your dose is being increased. These usually improve over time.
In the OASIS 4 trial, the most common side effects in people taking oral semaglutide 25 mg were:

Most side effects were mild to moderate and improved over time. About 3.4% of people stopped the medication due to GI side effects.
Tips to reduce side effects:
- Eat smaller, more frequent meals
- Choose lower-fat foods
- Stop eating when you feel full — this medication increases the feeling of fullness
- Stay well hydrated
- Avoid lying down right after eating
When to Call Your Provider
Contact your healthcare provider if you experience:
- Severe or persistent nausea, vomiting, or diarrhea
- Signs of pancreatitis: severe stomach pain that does not go away, with or without vomiting
- Signs of gallbladder problems: pain in the upper right stomach area, fever, or yellowing of the skin or eyes
- Signs of an allergic reaction: swelling of the face, lips, tongue, or throat; difficulty breathing; severe rash
- Signs of low blood sugar (if you also take insulin or a sulfonylurea): shakiness, sweating, fast heartbeat, dizziness, confusion
Important Safety Information
- Do not use this medication if you or a family member has had medullary thyroid cancer or a condition called Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
- Do not use with other GLP-1 receptor agonist medications.
- Tell your provider about all medications you take, as this drug can affect how other oral medications are absorbed.
- If you have a history of pancreatitis, discuss this with your provider before starting.
- This medication is not recommended for people with severe gastroparesis (very slow stomach emptying).
Storage
Store tablets at room temperature. No refrigeration is needed — unlike the Wegovy injection.
