The Zepbound KwikPen: Complete Guide

The KwikPen is not the same as the single-dose Zepbound pen. The single-dose pen is thrown away after one injection; the KwikPen holds four weekly doses in one pen — you keep and reuse the same pen for four weeks.[1]

Each KwikPen contains 2.4 mL of medicine delivering four identical weekly doses, plus a small amount of extra medicine so the pen can be primed before each injection — that extra is normal and expected.

One pen lasts 4 weeks. Do not throw it away after your first injection.

Your KwikPen comes in one of six strengths, and every dose from a given pen is the same — the pen does not let you change the dose:[1]

Check the strength printed on your pen label every time — especially if your dose was recently increased, so you don't use an old pen by mistake.

Before Your First Injection

• Get trained first. Your provider or pharmacist should demonstrate the KwikPen before you use it at home. If you previously used a different Zepbound form (single-dose pen or vial), you need new training — the KwikPen works differently.
• Vision problems? The KwikPen is not recommended for self-injection if you have significant vision impairment; a trained caregiver should give the injection.
• You need pen needles. They are not included in the box. Ask your pharmacist for compatible needles and confirm you have enough — one new needle per injection.
• Never share your pen. Even with a fresh needle, sharing can transmit bloodborne infections such as hepatitis or HIV.[1]

How to Use Your KwikPen

Always follow the illustrated Instructions for Use packaged in your pen box — the summary below covers the sequence:[2]

1. Check the medicine through the window: clear, colorless to slightly yellow. Cloudy, discolored, or particle-filled liquid means don't use it.
2. Wash your hands and wipe the pen's rubber tip with an alcohol swab.
3. Attach a new pen needle — brand-new, every time.
4. Prime the pen (flow check). Follow the exact priming steps in your Instructions for Use. Don't skip this.
5. Set your dose per the Instructions for Use.
6. Pick your site: abdomen or thigh; the back of the upper arm only if someone else injects for you.
7. Inject: press the button all the way down and keep the needle in your skin for the hold time listed in your Instructions for Use. Pulling out early can shortchange your dose.
8. Confirm delivery: the dose counter should be back at zero before you remove the needle.
9. Remove the needle immediately and drop it in your sharps container. A needle left attached lets air into the pen.
10. Recap the pen and store it in its original carton.

Your Weekly Schedule

• Inject once a week, same day each week, any time of day, with or without food.
• Missed a dose? Take it within 4 days (96 hours) of your usual day; past that, skip it and resume on schedule. Never take two doses to catch up. (Full rules for every GLP-1: missed-dose guide.)
• Changing your day? Fine, as long as at least 3 days (72 hours) separate the two injections.[1]

Write your injection dates on the pen carton so you know when your four doses are used up.

Storing Your KwikPen

Never freeze the pen — if it freezes, discard it even if it looks normal. Keep it in the original carton, away from heat and direct light.[1]

Throw the pen away at whichever comes FIRST:

1. 30 days total at room temperature, or
2. 30 days after your first injection, or
3. After all 4 weekly doses

This applies even if liquid remains — the leftover is priming volume, not a fifth dose. Write your first-injection date on the carton; that's your 30-day clock. Time the pen spent at room temperature before first use counts toward the 30-day total, so if it sat out a while, ask your pharmacist to help figure your discard date.

Traveling? Room temperature means at or below 86°F (30°C) — a hot car, beach bag, or checked suitcase can easily exceed that. Carry the pen in hand luggage in an insulated bag, out of direct sun. More in our storage guide.

Throwing Away Needles and Pens

Used needles and the finished pen go in an FDA-cleared sharps container — or temporarily a heavy-duty plastic container with a tight, puncture-resistant lid, kept upright and labeled. Never loose needles in the trash, never recycling. When about three-quarters full, dispose per your state and local rules; your pharmacist can tell you what they are.[1]

Troubleshooting

"Nothing came out when I primed." Repeat the priming steps per the Instructions for Use. If it still fails after the listed attempts, don't inject — call your pharmacist.

"The dose counter didn't return to zero." The full dose may not have been delivered. Don't give a second injection to compensate — you could take too much. Call your provider or pharmacist.

"I pulled the needle out early." Possibly a partial dose. Don't re-inject; call your provider. Next time, count out the full hold time before removing.

"I see bubbles in the pen." Small bubbles are normal and priming clears air out. Always remove the needle after each injection — leaving it on lets air in.

"There's liquid left after 4 doses." Expected — that's the priming overfill, not a fifth dose. Discard the pen.

"I dropped my pen." Check the body and window for cracks and the liquid's appearance. Anything damaged or off-looking: don't use it; call your pharmacist.

"It sat out of the fridge overnight." An unopened pen is fine at up to 86°F for up to 30 days total — note the date it came out and count from there. If it went above 86°F, call your pharmacist first.

"Can I use insulin needles?" Only pen needles your pharmacist confirms are KwikPen-compatible. Never improvise with syringes or needles for other devices.

"I switched from the single-dose pen — same storage rules?" No, and this is a common trip-up: single-dose pens and vials get 21 days at room temperature; the KwikPen and multi-dose vial get 30 days.[1]

When to Call Your Care Team

Call your provider or pharmacist if you:

• Aren't sure whether you received your full dose
• Can't get the pen to prime, or the button won't press down
• Think you took two doses, or dosed from the wrong-strength pen
• Kept the pen above 86°F (30°C), or think it may have frozen
• Are unsure whether your pen is still safe to use
